To register with the Practice you are required to complete both a GP registration form and new patient questionnaire form, these must be completed before you can get an appointment with a Doctor. If possible also add copies of your ID or proof of your address. Please visit the practice to collect the necessary paperwork, if you are unable to visit the practice, please contact us by phone and we will email the necessary forms to you if applicable.
Please be aware that we are currently not registering patients who already have a GP in the East Kilbride area. This is due to sustainability concerns and is with support of Primary Care Services.
The Practice accepts patients from the G74 and G75 postcode areas.
PLEASE NOTE ALL THE PRACTICES IN THE HEALTH CENTRE TAKE PART IN A ROTA FOR REGISTERING NEW PATIENTS, THEREFORE WE MAY REQUIRE TO DEFER YOUR REGISTRATION UNTIL THE APPROPRIATE WEEK (YOU WILL HAVE THE OPTION OF REGISTERING WITH ANOTHER PRACTICE WITHIN HUNTER HEALTH CENTRE DURING THIS PERIOD).
The receptionist will offer you a new patient appointment. Acceptance on to the list is as at the discretion of the Doctor.
Please obtain at least 1 months supply of any medication that you are currently taking from your previous practice as we may not be able to issue any medication whilst in the process of registering with us.
Patients are advised to the read the NHS counter fraud declaration before signing the registration form.
There are a number of reasons why you may not be able to register with your chosen GP, e.g. you may live too far away. If this is the case simply choose another GP in your local area.
Once you’ve been accepted as a patient, your medical records will be transferred to the Practice and you’ll be sent a new medical card.
Your medical records may take up to several weeks to arrive at the Practice.
If you default from your new patient appointment and do not inform the reception staff you may be refused acceptance on to the Practice list.
